CHDI School-Based Health Center
Located at 601 Dean St. - Enter through Crossroads Academy's main entrance on Park Street.
UCISD is committed to fostering a healthy learning environment for all students and staff through the establishment of the School-Based Health Center at Crossroads Academy Campus. In collaboration with Community Health Development, Inc. (CHDI), this initiative offers a convenient and accessible way to ensure the health and well-being of our community.
Please complete the School-Based Supplemental Health Services Consent Form. This form is necessary to grant permission for your child to receive medical treatment from a CHDI physician or healthcare provider through our school-based program, even in your absence. It's a crucial step in ensuring your child's health and well-being.
Signing this form allows UCISD staff to share your child's medical information with CHDI. This collaboration enables us to:
- Assess your child's health needs
- Coordinate appropriate care
- Provide necessary treatments or referrals
- Evaluate the effectiveness of our School Health Program
Once you have completed the consent form, please drop it off at the Central Office. We will promptly ensure it reaches the School-Based Health Center at Crossroads Academy.
For clinic visits on campus, you can walk in directly or call the School-Based Clinic at 830-900-7904. For appointments, please call 830-278-7105.
